To request a boy or girl's passport, you need the dedicated form, a passport photo and the signature of both parents, even if separated or not living together. The appointment can be booked online at the Police Headquarters or at the Police Station. It is a personal document: every minor must have their passport to travel outside the European Union.
The consent of both parents is required by law. If either of them cannot sign, an authorization from the guardianship court is required. The passport lasts 3 years up to 3 years of age and 5 years from 3 years onwards. Before leaving, always check the rules of the destination country: visas or additional documents may be required.
